Sasuke would probably win tbh, Itachi can't maintain his Mangekyou techniques (ie, Susano'o, Tsukuyomi and Amaterasu) active for a long period of time, and Sasuke has much greater control over Amaterasu, so I reckon Itachi wouldn't be able to beat him this time round
