Itachi has had his for years (4+) and in his battle with Sasuke he finally started to get blind.

Madara fought Hashirama for an entire day when he had MS, but he didn't get blind from it till later on in the story.

I didn't want to add Kakashi to the mix b/c then people would talk about him using Naruto's chakra as an excuse.

Danzou had Senju DNA which enabled Shisui's eye to be recover quicker after using KA which shuts off for a Decade usually.

Yeah Obito helped Kakashi there. Well he used it when he only had 3 tomoe sometimes but whenever he used Kamui he always had MS activated.

That's when Madara came back with Izuna's eyes inserted and new EMS.. Remember the battle shown before that was when Izuna was hit with Tobirama's Hiraishin , and Madara carrying Izuna back on his shoulder.
