What's the relevance of this? We are discussing medical ninjutsu and how is a Sakura pioneer, you claim Kabuto is a pioneer as well correct? So what does SM or him running an orphanage has to do with this discussion?

No logic! Tsunade herself established the NV medical ninjutsu system and each medical ninja in the medical system abides by her rules! Explain the logic of creating rules only to break them. And Tsunade's reforms only centered around putting medics on every ninja squad as a way to reduce casualties. It's referenced in both scans you posted. Not follow these 4 guidelines, that Tsunade herself deliberately ignores.
Tsunade only trained two students, Shizune and Sakura. That's a fact. That means all the other medics in the Leaf (you know during those decades of Tsuna not living there or practicing medical ninjutsu in the village) all had to learn from other people.
That's one definition, the word pioneer can be used in different context(s) It's most literally definition is a person who is among the first to explore or settle a new country or area. Among is defined as (1) surrounded by; in the company of.
(1) Sakura is among the first to master medical ninjutsu (unless you can provide a scan that dipictes someone other then Tsunade & Sakura mastering it), she actually is the first person to master each of Tsunades techniques and she is definitely the first to extraction poison via Dellicate Extraction Technique. (2) Sakura brought everything Tsunade taught her about medical ninjutsu into a new era (her generation) and established herself as being the worlds strongest healing ninja!
Aren't all medical ninja "masters of medical ninjutsu"? It's not an easy thing to learn, and they wouldn't just deploy half trained shinobi on missions if they didn't know what they were doing. CES and Yin Seal aren't medic techniques, if you think that qualifies for fully mastering medical ninjutsu.
