There are a lot of things about the clan that don't make sense, such as if Shisui was considered the most skilled of the clan how did Itachi kill him? and how did he manage to wipe out an entire clan, but I think most things will get answered soon.

Maybe Shisui was to trusting of Itachi always with his guard down, always relaxed around him. Also it is never said that Shisui had MS, Inane mistranslated the manga in to Shisui of the Mirage or Illusion when it was really Shisui of the Body Flicker "Shunshin no Shisui"
