Ok, at least now we have established that oro got the tools to defend against the powerful kamui and you're right that kishi obviously first prioty is how the story is being told so therefore he often waits for the character to use their trumph card.BUT that does not mean that in this case the kamui can't be defended by S level shinobis like pein,itachi, jiraiya, oro and madara.Does this mean he can`t hit it ever on any of the mentioned people? No, things happen and some of them could get sloppy but the majority of the time they would still be ready and deal with it.

Well, yeah it doesn't mean that and I know what S-class ninjas are capable of. But if we think just the real body and not clone, I can't think of way to defend against kamui since it's not like kunai which you can just swoop away.
But all of this is speculation. Can't possible guess any of those things until we see kamui in REAL use against high class ninjas.
