If you are referring to the image Hiruzen saw before he closed his eyes, that was an image of Orochimaru when he was a kid, if you are referring to the hysterically laughing Orochimaru kid, that would be the clone body he inhabits.

How would he be both, unless he goes into a hermaphrodites body then he cannot be both genders. Genders are defined by the parts you have. Regardless of WHAT you feel you are. Orochimaru is just implanting himself into the body of the person, if he goes into a females body he will still not be able to reproduce with another female it just wont happen thus he would be female at that time, if he goes into a guys body he is a male. End of story. Lmfao.
