People put too much stock in the fist fight, the battle was more than just that, the fist fight was just Sasuke wanting to kill Naruto.

There was emotions, ideals, and ego also on the line, and Naruto won all the above. So while the fist fight left them both in the same condition, the overall battle is Naruto's victory.
