Itachi isn't an idiot, did you see him Susano'o spam Kakashi, or Jiraiya in part 1? He knows how to use his techniques effectively, unlike MS Sasuke who spams, burning up too much chakra.

Itachi is smart enough to force him to waste MS techniques giving the fight to him.

Even if this was sick Itachi I would still give this to him extremely high diff.

just stop. if he doesnt use soosano right off, an arrow one shots him as sasuke wastes no time bringing it out.to fight and hang with sasuke, he will need to use soosano and his ms techs from the off or else he will get fodderized as simple as that. all other are techniques are worthless so its a soosano battle , if he is healthy he could outlasts, if not he loses
