Itachi wins, high diff.

He outclasses sasuke in basic skills, speed, strength, taijutsu.

Once MS comes out itachi has sasuke beat. Itachi was able to maintain susanoo up until the very moment he died, sasuke went blind but had to deactivate it, so itachi can tolerate susanoo better than sasuke.

Totsuka & yata also give itachi the advantage in terms of firepower & defense, so sasuke doesn't have a way of breaking susanoo.

just stop sasuke is physically stronger than itachi speed would not play much of a factor because their so close sasuke was much more blind than itachi his eyes were dieing itachi was just sick and no where near as blind as sasuke so no totsuka is useless cannot pierce it yata is aswell since they both cant get past the standard susanoo

Sasuke takes this - he has better Shuriken-jutsu, added Kenjutsu, better ninjutsu, at speed and reaction rates they are equal. So at base skills level, Sasuke has better skill repertoire. Once Susano'o goes up, he outlasts Itachi. He will just have to bear his attacks till Itachi's Susano'o loses tiers. This the fight is his.

Itachi isn't an idiot, did you see him Susano'o spam Kakashi, or Jiraiya in part 1? He knows how to use his techniques effectively, unlike MS Sasuke who spams, burning up too much chakra.

Itachi is smart enough to force him to waste MS techniques giving the fight to him.

Even if this was sick Itachi I would still give this to him extremely high diff.

Izanami wouldn't work against Sasuke.

Izanagi is iffy.
