Actually, considering Itachi's reserves, Sakura could prove to be beneficial.
She's already been shown to transfer chakra to Obito, so she could do the same to Itachi who could then spam clones and Amaterasu.
Well, this is healthy Itachi so he should be fine, but with the opponents, he will be overwhelmed, especially with a speedster like Gai and Jiraiya's agile moveset. Sakura will barely be able to handle Tsunade, but in the end I think that Tsunade will out best her.

Team Gai should have this

Jiraya beats Itachi, and Tsunade beats Sakura.

No need for Gai. Gai would oneshot all of the guys in the battlefield, and would die himself. All of them lose.

You didn't mention Itachi starts with a build up in chakra in the eye, so tsukuyomi is not happening before Gai avoids eye contact(like he usually does while fighting Sharingan users) and blitz him wih evening elephant

The ability to surpass body's physical limits is different than the ability to surpass the limits of the mentality in the body; that is, the brain. Blood flow, speed, strength and increased overall power is irrelevant in front of brain's capacity to react and process events and experiences. Understand that the ability to process an image is dependent on neuron signals. Remember that this also applies in Naruto...

The speed required for one's nerve signals to command a muscle takes up about as long as one needs to process the image. However, when Gai would already be moving, Itachi's fate would be already decided; unless your point is that both have eye contact at the moment the fight starts, which still leads for Gai's win but with consequences.
Hmm...good points collectively, to which I agree since I overlooked that myelination increases the velocity of the nerve impulse to more or less that amount.
