Whilst you're correct about editor notes in general, this particular case is none.
It is Sakura's interior monologue indeed; just like it has been throughout the entire chapter.
Thoughts aren't solely indicated through bubbles. What to make of it, though, is an entirely different pair of shoes.
To believe, however, that an editor would refer to Sakura using the pronoun I and subjectively adressing Tsunade as sama is but foolish.
