Has anyone ever unlocked MS in only one eye? Show a scan if so, because they haven't Both eyes change at the same time when they evolve to MS.

Obviously not,you just need to unlock the powers in both eyes in order to use Susanoo.Think of it as one sharingan has 2T while the other has 3T,you can awaken a ocular power in one MS eye while the other is still locked.

I like where you took this, even if not true, i do think that madara's card has to do somehow with his sharingan/rinnegan techniques/abilities.

After Recent chapter. I believe it is quite plausible that there is some sort of spirit technique that Madara used to keep Sasuke alive as well.

Shishui and obito never had Susano'o. Or, at least, it's never hinted they did. Seeing as they didn't have ama or tsuky, the fact they didn't have susano'o would strengthen the point you need ama and tsuky.

As for madara, nothing says he doesn't have ama or tsuky. Itachi's tsuky is a special brand which completely breaks logic, while sasuke's tsuk is simply an enhanced sharingan genjutsu(EMS genjutsu), same as with madara's own. In any case, madara's shown the ability to use a tsuky close to that of itachi after he siphons off gedo's power into his body.
We don't have any reason to believe he doesn't have Ama. He's a katon master.

With this being said I believe Madara may be able to bring back either Rin or Sage of the Sixth Paths as his triumph card.
