Like I said, Sasuke's speed and reflexes were far greater than Madara's at that point. He cut down Madara and Madara couldn't even complete a thought during the process.

How was it that someone much faster than Madara was unable to notice Madara slipping out?

madara already subbed with his limbo and naruto had it under control meaning no more subbing. and people never see a substitution coming, you think deidara while he was watching sasuke saw it coming? no
