1) Unspecified cause if death: died presumably in war based on what I remember being stated.
2) He died for two reasons. A) a gaping hole was blown through his stomach from Kurama's claw and B) He used a jutsu that seals his and an enemy's soul.
3) He died for two reasons. A) He used a jutsu that seals his and his opponent(s) soul and B) he was stabbed fairly close to the heart.
4) He died for a series of reasons. Firstly, he'd already been fighting Kakashi, who immobilized his hands. Then, he was emotionally unstable from Haku's death. Finally, combining the first two, he recklessly charged through a group of mercs with just the intention of killing Gato. Being priced by multiple blades melee weapons. He bled to death.
5) He died because the Rinne Tensei costs one's life to use.
Neji Hyuga died because 4 Mokuton Spears plunged into his body and each ruptured into four different directions. This action virtually destroying and rupturing his insides. He subsequently passed on due to a combination of organ failure and blood loss.
