Well, Hashirama is the creator of Kage, so his definition and ways of running the village are correct, technically-speaking.
Sasuke can't just up and take someone else's creation and change the meaning of it.

Sasuke didn't change the meaning of Hokge. Him and Hashirama both acknowledged Itachi as the superior shinobi.

Referring to those who protect Konoha in the shadows. I suppose either Madara or Danzo would be the first because both took aggressive measures to ensure Konoha's supremacy.
However, Itachi was the first in my book. He used noble and just methods in looking over Konoha. Then Sasuke's second. And Boruto will be third if he follows through with his declaration to Sarada at the end of his movie.

Can't think of any "Shadow Kage" from other villages. No other villages have produced an Itachi or Sasuke type of shinobi.
