"Shadow Hokage". Not creative, but makes sense.
Referring to those who protect Konoha in the shadows. I suppose either Madara or Danzo would be the first because both took aggressive measures to ensure Konoha's supremacy.
However, Itachi was the first in my book. He used noble and just methods in looking over Konoha. Then Sasuke's second. And Boruto will be third if he follows through with his declaration to Sarada at the end of his movie.

Can't think of any "Shadow Kage" from other villages. No other villages have produced an Itachi or Sasuke type of shinobi.

Those shinobi want to protect the village in a different way, other than just running it from the light, or doing missions in support of the Village. They want to do the dirty work, whether it be silencing threats that no one hears about, or going on long journeys away from their families in order to protect the things they care about.

I wouldn't call it 'shadow Hokage', since they aren't literally running the Village. I would just call it being a shinobi in the shadows.

Not really. For every evil action, like having the Uchiha massacre, or making Shisui kill himself, had a good cause and result in mind behind them, such as protecting innocent villagers. Danzo supported the massacre because the Uchiha were criminals aiming to cause harm to innocent people. His support prevented that. Danzo is one of the most realistic characters.

Danzo figured it was best to kill the innocent Uchiha as well, just in case they desired vengeance after the deaths of the guilty ones. There is some logic in that, but it's still twisted.
As I man, I'm huge on the protection of women and children.
I just can't get jiggy with villians. I'm a fan of pure, one-dimensional do-gooders.
I see you like Doflamingo, so I doubt we could ever see eye-to-eye in this matter, which is fine.

Shisui couldn't handle the heat of everything that was happening. His suicide meant something like .. "Screw this, let them handle this crap.", he leaves a suicide note, his left eye to Itachi and just disappears.

Danzo left him no other option. Shisui committing suicide and leaving the rest to Itachi was the most helpful thing he could've done.
He even made sure no slimy bastard like Danzo or Orochimaru could discover his body and utilize his DNA. #ShisuiTheMatyr

He is the True Hokage.

Well, Hashirama is the creator of Kage, so his definition and ways of running the village are correct, technically-speaking.
Sasuke can't just up and take someone else's creation and change the meaning of it.
