1> Amaterasu and Kamui are cast the same way through the eye, when Kakashi used Kamui it shows his line of sight as he targets Deidara and begins warping him, Same with how against Raikage, it shows Sasuke's view of A as he targets the flames at him (The difference being of course that A is far faster than Deidara and is aware of Amaterasu) There isn't any "prep" time for these occular jutsu, just however long it takes to activate MS and set your gaze on your target.

Stop underrating kakashi damn it , also he can use various tech's to attack itachi / sasuke , unless they hide inside susanoo , which would take alot of chakra , so kakashi could just wait and surprise them with his attacks .Also if kakashi can get his blood to enemy he can summon ninja dogs and bite/stun enemy .
