Itachi wins he knows the sharingan in and out. He's also a more intellegent fighter than Sasuke and knows how to conserve his chakra instead of spamming Susanoo. He has the same moves, he probably understands the weakness of the jutsus better than Sasuke as well.

Sasuke wasn't fighting itachi on "nearly equal footing", itachi was much weaker than he normaly was even zetzu said and he was on deaths door and almost blind.

Even then itachi could of killed him if he wanted to.

It all depends if sasuke has the spirit weapons itachi had. If not then he is done for, If he does then he wins.

Itachi wins. Sasuke like naruto might have become more powerful than him in terms of sheer power/chakra level, but itachi is technically still a far superior ninja than both of them.
