Well any MS user has the potential o unlocking Susanoo and all it's forms including PS even if he doesn't have EMS, while there's no proof of Itachi reaching that level it's not unlikely that he actually achieved it but couldn't use it either because of illness or he didn't had to use it in his battles cause he had other options to deal with his enemies.

So far the only people to unlock PS were Indra's reincarnations and an MS user with Rikudou's chakra.

Itachi fits into neither category.

Itachi's illness was caused by Susanoo, just like how Sasuke started coughing up blood and falling over after he got Susanoo.
