Also in Japan, gay relationships are very rare and taboo because of the traditional families and stuff so maybe that's why Kishimoto didn't actually reveal that it was Shisui. But who knows? It could be a woman. It's just that Shisui is our best guess since we didn't get to see so much of Itachi back when he was young.
