Last I checked, the whole fight was Itachi playing Daniel Ocean from Ocean's Eleven with Sasuke. Had he fought 100% AND had he not been sick AND had he NOT loved his brother, Sasuke'd been kissing the center of the earth since loooong ago.

Try to keep in line with what Kishi has shown us so far. After Naruto beats Itachi, he is still being hunted by the Akatsuki. Pain would still kill Jiraiya, and Tobi will have an easier time swaying Sasuke towards the darkness. Sasuke wouldn't have his MS. Anything else?
