Because Genjutsu doesn't do any physical damage, and in a bloodlusted match up, the participaters usually go for the OHKO

Itachi only way to deal real damage is to catch him in genjutsu first. Seeing as Katon is tanked then he heals from it. same goes for regular Kunai's.
