Kakashi..it's already been decided that if Tsunade were to die, it goes to Kakashi. He was even chosen over Danzo until Danzo convinced the elders(put them under a genjutsu I think) to choose him instead. Then after Danzo died he was chosen again, til Tsunade awoke.
