The slowpig is right. Kishi has already confirmed it was an STD, but we don't know it was HIV. It might have been syphillis slowly addling his brain over the course of years.

We know he didn't die of old age...he died during the wars that broke out shortly after the formation of the Shinobi village system. It could have very well been the onset of the First Shinobi World War

It would be a huge discredit to all of the hype to both Hashirama & Madara and their legendary rivalry if Hashirama was defeated by anyone else

More likely he either died one of 2 ways: 1) overly using the regeneration shortened his life, or 2) he sacrificed himself

I seriously doubt someone killed him in an actual battle. If Hashirama was the "God of All Shinobi" whomever killed him would be known throughout the world
