I always wonder this about top 10

Top 10's are based on one's ability to defeat others. Tsunade cannot defeat many people, at least not those who do make it into top 10 lists. I don't see your confusion. You clearly know what they're based off of, but don't understand that her ability to heal does not come in to play in a 1v1 fight.

Eh, you have a bad habit of taking things out of context so I will say exactly what I mean. Top 10's are usually based on who can beat who more times than losing to that same person, characters who can beat one another, and characters who would win the majority of match ups they were put in as opposed to other characters in those same match ups.

I base them on prowess. Tsunade is a medical ninja, why would i base her on the ability to fight. And if talk about the ability to fight. She did something Hiruzen needed paper bombs for. And not forget destroying a ribcage susano'o. And woopin complete susano'o.

While I determine it on pure prowess in what that person does, you guys determine it by who can beat who in a fight.

Poison.. umm she kind countered poison in the second war.. she kinda blitzed a Orochimaru who was looking right at her. She then solod Manda.

These scenarios usually exist without prep like you're giving Tsunade up there.
Just accept that most people judge things differently than you and you'll be okay.

Most scenarios, my point is exactly that. Itachi can win in many scenarios against Hashirama and Madara. See now you changed your definition of top 10 to the most wins out of a scenario.
