Creation of all things. Since Naruto possesses the tailed beasts' power and Kurama's yin chakra, wich is a huge amount of yin chakra. I don't think he just has yang release.

EDIT: like Retsu said, the sage used chakra to create the bijuu, so I think chakra is needed for COAT to work. He must've taken Kakashi's chakra and used it to recreate his eye.

Firsty, senjutsu of the six paths contains both Yin and Yang, since before gaining control of this power Obito couldn't use Omyonton but as soon he gained control and had all the gudoudama orbs surrounding him, he started using Omyonton !!
Secondly, rinnegan also consists of both yin and yang.....and since Sage gave powers to both Naruto and Sasuke, it wouldn't make sense if he gave yin- yang to only Sasuke....!!
So although Naruto and Sasuke may represent Yang and Yin individually, they have both of these powers!! U_U

He is not doing an eye surgery, he can create things with chakra, not perform surgeries.
