Am I ignoring it? No. Did Hiruzen get a heart attack? No. Note what Hiruzen says, "You're always the same, Hashirama-sama." He wasn't surprised or scared. He was most likely only worried. Why do I think that he wasn't scared? Look at Minato's reaction, he wasn't scared despite the incredible power of Hashirama, so it makes no sense that Hiruzen would be scared when he knows what Hashirama and Tobirama are capable of.

If you're so convinced Hiruzen was scared or surprised of Hashirama's power, that doesn't go logically with him knowing Hashirama better than anyone else, bar Tobirama. Logically, if he does know Hashirama and Tobirama better than anyone else, he wouldn't be scared. More likely than not he was worried, but since you're so convinced of Hashirama's superiority, why have you continued to ignore that Hiruzen implied he is the strongest Hokage? That's where your case for Hashirama doesn't persuade me. If you want to persuade me, explain to me why Hiruzen stated to Iruka to not speak in past tense, directly stating he is the strongest Hokage even at the age of 69.

With the return of Hiruzen and the fact that Hiruzen destroys Mokuton easily! I wouldn't be surprised if Hiruzen's feats put him on Hashirama's level. Even his hype implies so!

Hiruzen was called the God of shinobi cuz he was a skilled ninja, knew a lot of jutsu, and was the strongest of his generation.

But since he's an old man right now, he has LONG lost his main power. Which puts him as the weakest. No one knows his full strength so we can't judge what place of the Hokage he was in his prime. We can only rank him in his old age...which people normally put as 5th.
