You can clearly see that Madara isn't getting hit. Look at the last kick he threw, Madara dodged it and Gai ducked down to use Hirudora, and failed. You can speculate on what those impact marks are (Probably just an effect added for Gai's punches) but one thing I know for sure, 100%, is that Madara clearly evaded all those attacks.

If Madara was getting hit he wouldn't be jumping backwards uninterrupted, you'd see some flinching.

But the manga scan shows that Madara was evading all of his attacks. Contact couldn't have been made. The last one shows the effect there, but it shows Madara jumping back, not blocking.

Tobirama can also create Shadow Clones to clone feint Gai. We all know Gai is not the sharpest knife in the drawer.

Depends on the situation, but considering his knowledge on teleporation, and Tobirama's status, it's safe to assume he'll go to the extreme and handle the situation accordingly.
