I would say Tobirama.
Founded the infrastructure, and made the best of a bad Uchiha situation. Protected Konoha with his life, made techniques that make Naruto and Minato famous. He taught Hiruzen and trained with Hashirama. He was an all around great Hokage.

Hashirama failed to kill Madara and you see how it is now.
Tobirama killed Izuna, made the Uchiha feel like they didn't belong by placing them in a region within the village. Created the original reanimation jutsu... In which was used on the 3rd.
Hiruzen let Itachi take out his own clan, allowed the boy to ruin his life and rep. Was sensei to Orochimaru in which was one of the greatest villains this anime was blessed to have.
Minato failed to save Obito and Rin. Madara took advantage of the situation and took Obito off his hands.
All four of them are responsible for for what has happened in this war.
Tsunade wins in my opinion. She has not messed up to the point that loads of people are dying for. She made it to that all teams have to have a medical ninja on their team. She has brought healing wounds onto a whole new level.
Don't get me wrong, she is my least favorite out of the Hokages but she didn't mess up like them.
