Actually I think that was just pure luck, Sasuke, Naruto and anyone else had no knowledge that Madara wanted Hashirama's SM. Sasuke only wanted to kill Madara and was trying to slash him up which Madara evade and when Sasuke went for a stab Madara blocked it. He was saying Die after all

. Plus Madara has his eyes close and still evaded and blocked Sasuke's attacks, he is alive and isn't going to let someone stab him in the body as he would when he was an edo, people seem to forget that (not saying you, people in General).

Naruto has more knowledge on Madara (Susanoo, Rinnegan and it's jutsus, wood jutsus, strong katon) from when his clone fought Madara with Gaara's and Oonoki's group and facing Madara with Kakashi, Guy and Bee when he joined Obito. That's why he told Sasuke it's pointless to just throw jutsus at him. Sasuke EMS does allow him to get in Madara's face and allow him to read movements to analyze his movements but attacking from mid range to long range to analyze Madara's fighting style still seem to be a safe choice but Sasuke does have his eyes to help him so I'll give him points but nothing shows he had any knowledge of Madara taking SM or trying to hit Hashirama's face but to merely go for a kill shot.

Throw out a couple of wild attacks, all the while waiting for the perfect moment to strike the soft spot.

You forget that Sasuke have to actually active the Sharingan's ability to see chakra as he did when trying to see where Kabuto was, it's not active unless they make it and Sasuke had no reason to have it on when Madara was right there with his eyes close so I don't see what theory he could have been trying, especially saying it was their only shot and saying die.

I like the ending of your statement d) my theory could be right. My other point, don't be a fool once the sharingan is activated, which it was. The user doesn't have to do anything special to see what chakra is doing in another persons body. The only time chakra is not visible is when the Sharingan is not activated. Madara now has two different types of chakra circulating through his body, and Sasuke can see that. Lastly it may have been a mistake that Sasuke discovered Madara weak point, but I can guarantee he Noticed Madara cautiously evade that final attack. Which just so happens aim at Hashirama face.

P.S. The way Madara was angled, there's no way Sasuke could have pierced his heart. Rather Hashirama face would be ripped off. Like a bump being burst by a needle.

Except that Madara can absorb chakra, so that wouldn't have worked.
