I was wondering if he combined Gear 3rd with it, or he just blew more air into his muscles enhancing gear 4th. Its a bit hard to tell from the one scan.

I think it would be just safer to say it is an enhanced Gear 4th.

damn, no because the next number should be for the next tech. that's why I didn't say this was gear 5 since it was a combo of the 2 gears I figured I'd add them but I didn't remember him doing that at TB. Shoot, should we multiply them then? Gear 6 and Gear 12?

G2 and G3 combined almost look like G4 in concept. Strength and speed. But this combination of G3 and G4 apparently is all what Luffy has got. Blood stream, bone, muscles. There's nothing more.
