1. Well, you'd only use this jutsu after trapping/covering the opponent in sand. Cause it would be useless if done otherwise.
2. How? Like I said for 1 (these questions are quite similar if not, the same) - you have to find a way to cover your opponent with your sand whether it be his whole body or only just a part of it to make sand burial useful. So, you should bait your opponent into getting covered in your sand (first step is to make him touch it e.g. change the terrain into a desert, bombard sand attacks at him, etc.). Once he has some of your sand on him, you go for the kill. Ofc, the more sand covering him, the better.
3. Personally, not getting the sand anywhere near you is a good option. When I fight a sand user I always try to not get near their sand, stopping them from even conjuring up sand from underground and just stopping them from using sand jutsus in general is what I'd do (keeping them on the back foot). But if their sand is around me, taking flight is an alright counter but finding a way to blast back the sand (away from you) would be a better idea. Basically, finding ways to nullify the opponent's sand altogether i.e. using water, oil, dense metals/materials to make the opponent unable to manipulate the sand altogether.
