Alright, lets say every village goes back to normal after this war ends because picking one person to unite the whole goddamn ninja world would be problematic. For Konoha at least, this is how i reckon it'll be run.
HOKAGE
Hokage must be the strongest ninja in the village. At the very least, they must be on par with the other Kage of the 5 great ninja hidden villages. The Hokage is not only a political leader, but they are also Commander in Chief of the armed forces of the Hidden Leaf Village; as such, they must exude charisma, wisdom/experience and instill a certain degree of fear in order to deter the armed forces of other villages. Lack of power or wisdom could consequently result in a war, as the other Kage can sense fear or weakness however small it may be.
Candidate: Naruto Uzumaki
At the age of only 16, Naruto has proven that he is extraordinary leader. He has experienced countless battles and won with odds heavily stacked against him: he has defeated some of the most powerful ninja in the history of Narutoverse: among these legends is Nagato; who himself was leading a group of individuals capable of waging all out war with any of the Major Hidden Villages. Through his experiences in several S-class missions, Naruto has gained extensive knowledge of several hidden leaf villages, and frequently encountered Village leadership; as a matter of fact, he has developed a powerful friendship with the current Kazekage and the 8 tails Jinchuuriki; from whom Naruto has gained lots of wisdom/knowledge, conversely, they have learned much from Naruto. Within the Leaf Village, Naruto has formed powerful friendship with several future Jonin, the most powerful of whom is Sasuke of the Uchiha clan and awesome medical ninja: Sakura.
According to Kakashi, team 7 (Naruto, Sasuke, Sakura) works best with team 10 (Ino-Shika-Cho), who usually reinforce team 7 on their missions. This partnership between these two great teams is absolutely essential in the future of the Hidden Leaf Village. The chemistry between these two teams is absolutely fantastic: starting from Sakura and Ino, the two are brilliant and smart medical ninja with a wide range of crucial abilities which will be needed due to way in which Naruto and Sasuke go all out in war. If Sakura has a contract with the slugs, she can communicate effectively with the brains of the two teams: Shikamaru, who in turn can direct the operations conducted by this elite team.
Chief among Naruto's ambitions as Hokage is peace, but he will start by changing the system of the hidden leaf. He has promised he will change the Hyuuga clans tradition of marking inferior members of the clan with a curse. He will probably reverse the law on Uchiha's and possibly give men like Sasuke a lot more power within the village council and reinstate the Uchiha police force.
Jonin Commander
Jonin Commander is the second highest rank in the village, between regular and special Jonin and the Hokage. The Hokage often delegates a lot of duties to the Jonin Commander, such as assuming strategic command of the forces of Konoha and taking direct command of the forces, whenever the Hokage is unable to do so. The Jonin Commander is also part of the council and therefore plays a major role in political and military decisions.
Candidate: Sasuke Uchiha
Sasuke is a born leader, who confronts the enemy head on. I've heard there are fans who want Sasuke to be head of root because they somehow believe it's symbolic to recreate the 'leaf' and 'root' relationship that Hiruzen and Danzo had, but to me that does not make sense:
Militarily, it's a poor use of Sasuke's powers, abilities and vast reserves of chakra;
from the moment they became a team, Naruto and Sasuke have worked together perfectly in battle.
Sasuke's natural leadership skills and charisma would keep all the Jonin in line. Furthermore, he is known to command powerful ninja while he led Hebi/Taka. I believe it was Sasuke's life long ambition to be part of the hidden leafs police forces. He would be the first in line to be Hokage, as well as assuming Supreme Command of the Uchiha police force. As his title is Jonin Commander, he will naturally have more power than other Uchiha's did.
Department of Medicine
We all know the critical importance of this division within the 'cabinet' or council of the Leaf Village. Tsunade's one desire for this department was to incorporate a system in which all ninja teams have at least one medical ninja who's in support of the front line fighters e.g. Naruto, Sasuke, Lee. Obviously, the head must be highly intelligent, and received training from the worlds greatest medical ninja: Tsunade. In times of war, the head of the DoM should be capable of mobilising summons to every corner of the battlefield, with perhaps the slugs.
Candidate: Sakura Haruno
Sakura has conducted operations on poisoned ninja successfully and managed the village hospital during the Pain arc. Sakura has effortlessly defeated a white Zetsu, helped in dissecting the body and developed a theory about Madara and Zetsu. Furthermore, she has seen just about every wound imaginable, and treated almost every wound we have seen. Moreover, Kakashi claims Sakura could surpass Tsunade in medical ninjutsu due to her intelligence, age, experience, and other abilities which Tsunade probably does not have.
Sakura is not as developed as some of the other characters, but she does have the potential. What she needs now, is a means to communicate with her team during war, with slugs as well as the means to mass heal thousands of injured ninja. In addition, she would need to communicate with Shikamaru and Ino, possibly with a telepathic link between them using Ino's powers. This method is better than the slugs, since they can communicate privately without the threat of their plans getting overheard.
Head of Root
Root is a subdivision of ANBU, who are supposed to work with the Hokage to eradicate any threats to the Hidden Leaf. Its top priority is secrecy, hence why it requires men like Shikamaru and Sai. The new head of root must not be selfish or arrogant, otherwise they are worthless to the village. The head of root must have the skills to choose accurately who to eliminate and have the necessary strategy to do so as efficiently and covertly as possible.
Candidate: Shikamaru Nara
Shikamaru has consistently provided the best strategies for the teams. As a result he has been 'promoted' to Chuunin before everyone else; and during the ninja war, he was promoted again to General and proxy of Gaara. I can see him doing the same again for the hidden leaf village to a much lesser extent with Sasuke there for Naruto.
Shikamaru does not have the levels of chakra comparable to men like Sasuke or Naruto. Besides, Shikamaru works best with Ino and Chouji, as there is a lot of history of the Ino-Shika-Cho clans working together. With his clans shadow manipulation powers and their generally high levels of intelligence, they could conduct secret missions on Naruto's orders.
But here's where Ino comes into the picture. Unlike Danzo, who captured threats and usually killed them, Ino could gather a tonne of highly valuable intel from the ninja's minds; while Shikamaru held them down. Then, Shikamaru and Ino can jointly analyse the data they extracted, from which they can formulate a strategy and pass on to the commanders in order of importance: Naruto > Sasuke > Sakura, etc
This strategy will be efficient and keep valuable ninja like Shikamaru from harm. If Shikamaru is caught or killed, it would be a massive blow to the hidden leaf. We know he's not as powerful as monsters like Naruto and Sasuke, so it's best to keep him out of the battle field unless it's absolutely necessary.

The intelligence division is tasked with analysing intel they have obtained, whether it's the form of objects or a person/ninja. Inochi Yamanaka (Ino's father) is current head of the intelligence division. In order to be effective, the head of this division must be able to project their minds into others either for communication or intel extraction. Finally, the intel division must work with root and other departments and relay intel as rapidly as possible, otherwise it doesn't matter how much intel they have gathered. Think about it: they discover intel, but can't analyse it as well as men like Shikamaru or relay it rapidly enough to the front lines where Naruto is fighting the strongest enemy he has faced since Madara - it would be epic fail for the intel division.
Candidate: Ino Yamanaka
Ino just like Sakura is not really a front line fighter, but she is useful in other ways: medicine and intel. Ino's relationships with Sakura and Shikamaru will be paramount in her success. When they capture a intruding ninja, Ino works on the minds and Sakura works on body: all 3 of these intelligent ninja together can help Naruto win fights.
