The most common medium is a scroll.
Fuinjutsu works by creating Sealing Formula's with chakra infused in them, and then sealing that object inside the sealing formula. Naruto managed to summon a toad to the battlefield by using a sealing formula.
Because seals are fueled by chakra, they become weakened through time, as the chakra that was expended to create and maintain it, becomes consumed. Because of this, when sealing something in a living being, the seal is generally made to use the chakra from the being to sustain itself, delaying the eventual weakening.
Fuinjutsu can be used, mostly to simply seal an object in inside another object, however the user can use this to store chakra on a tag with a sealing formula, then use it to renergize yourself after a fight, seal away the ability to mold chakra, or even use it to create homing rockets out of kunai by simply charging chakra into the kanji's. Fuinjutsu holds the most potential out of all core arts(Ninjutsu. Genjutsu, Tiajutsu)

Sealing seals objects inside of a scroll. If you don't have the necessary tool within a scroll, say a shuriken, you will not be able to unseal it because it was never placed into the scroll. While you can SUMMON shuriken from other places, you can only UNSEAL shuriken you have sealing tattoos/scroll for.
