So basically,

if I see a hot chick I say "comment ça va?"
if I see an old man having a stroke, coughing blood, I say "ça va?"

"Ca va" is less formal. Use it with friends, not with employers, etc.

You can also use, comment allez-vous?
But that's more formal than the other two.

This guy knows what he's talking about. "Vous" is used to signify respect, as opposed to "tu", which is informal in comparison.

If I was a teacher I would say you have 4 errors in that single phrase.
OT: Yes, learning french is very hard especially if you know english. It is easy to mix english and french to make a lot of errors.

heheyheyheyhey since this turned into a conversation rather than a simple question/answer thread, i'd like to know what's wrong. and if i pointed out the things that are wrong correctly.
so number 1: you type m'apelle, with this ' shit
number 2: apelle, not mapple or mapelle
number 3: he ought to use "tu apelles" (i think i'm wrong with this one :\)
number 4: he should have used the "et" before "tu apelles?"

The phrase should have been:
"Bonjour, je m'appelle Maleque. Comment t'appelles-tu?"
