Why didn't Madara and Izuna just switch eyes so they could both have the EMS or why didn't all Uchiha just use Tsukuyomi on each other to make them think they killed their best friend so they could gain the Mangekyo

Good point. I wonder why nobody has done that. There has to be some reason why that wont work.

Because one of the eyes is completely blind... izuna or another Uchiha taking their brother's eyes would be pointless do the fact their brother's eyes had zero light in them.

You need to have both eyes in order to unlock the full potential of the sharingan. Madara said this himself. It would not have worked and besides, Izuna wanted Madara to protect his clan and their honor which is why he sacrificed himself. Also not every Uchiha has Tsukyuomi, Shisui and Obito are examples of that. I'm sure you are also aware that even though, Tsukuyomi and normal sharingan Genjutsu are similar, they also have their differences.

I believe I read somewhere in the official databooks that to get an eternal mangekyou sharingan you don't just 'trade' eyes, the eyes must be 'fused' together to create it so one pair of eyes would go with the other to give the stronger Uchiha EMS, that is why they killed eachother. To take them.
