The 'trade' idea seems logical but not the Tsukiyomi idea. 1. Not every MS user has Tsukiyomi 2. The person has to actually believe that their friend is dead. If they know it's a genjutsu, there's no point.

It plainly states there that it is the donor's eyes are what become eternal, not both eyes. At no point was it ever suggested that the eyes fuse together.

As for the question, Madara and Izuna never traded eyes, because they had no way of knowing what would happen if they did. Izuna also died, so giving him Madara's original eyes wouldn't have made sense.

He died from the wound Tobirama gave him; the whole reason he gave Madara his eyes was for the benefit of the clan. Trading eyes in that situation would have made absolutely no sense.
