In That Fight sasuke had, His own Powers+Cursemark+Oro's Powers (since he absorbed him.
A healthy Itachi would of Beaten beat 3tomoe Sasuke with Low difficulty, It wouldn't be as Quick as the Itachi Vs oro Fight no, But it wouldn't Take That long.

who has more impressive feats then beating deidara taking down a thousand shinobi without killing them? Killing orochimaru and "beating" il admit itachi was sick and plotting to help sasuke but the fact remains, sasuke is wayyyyy stronger then he was back then. itachi in his prime wouldnt be able to out last sasukes superior amaterstu spamming and enton release. since we have seen sasuke beat tsukiyomi with a 3 tomoe with an ems it should be light work (wale voice) then it would end in a sussano battle an ems is going to be the deciding factor there when itachi is in pain because of the technique sasuke will take advantage and end it. :scorps:

You forgot to mention Sasuke had Orochimaru's abilities and the Curse Seal and Itachi was holding back and he was severely sick, so yeah.

Ms itachi > ms sasuke
It maybe confusing but itachi has more experience with ms & the mirror while in base sasuke has some op moves
