I've seen your posts, and this might be the only time I will ever agree with you, but yeah, Itachi is the most likely candidate.

Assuming that the 4th still died and all that, if Itachi remained (publicly) loyal to Konoha, then I could see Sarutobi stepping down and letting Itachi take the reigns.

if it wasnt for the coup de tat and madara always wanting to go to war then yes there would have been, i mean come on the uchiha is like tied for first in leafs strongest clans. look at past hokages tsunade and danzo,really, poeple dont think itachi would b suitable or shisui. i believe you take away madara and there could have been at least two uchiha hokages, tobirama wasnt all that strong and its possible he could b sustituted for a uchiha hokage and tsunade could have been substitued for fugaku or itachi
