Kirigakure was the only of the 5 great nations to not get involved in any of the previous Great wars and Kiri was said that they didn't include themselves in the conflicts of the main land and usually kept the themselves. Also the akatsuki was rumored to be formed by Kirigakure and Madara being the ring leader could explain.

Well, Madara was said to be dead, so no one would suspect him of being alive and especially being a Kage. He may had changed name but I guess his face was not hidded or he could have his face hidden but shown it to the first Akatsuki members (since I read in this thread that it is said that Akatsuki could have been founded in Kirigakure), or to Kisame, because he was of that village.

It is confirmed that Yagura was indeed controlled by an unknown genjutsu as Ao (?) saw with his Byakugan. But we don't know if it was Madara. He could be nevertheless.

Well , i think Madara didnt showed himself as the Mizukage , i think he was controlling Mizukage from the shadows , and that some ppl knew that.I think the two of them never met face to face and that Kisame didnt knew how madara look like. And when he took off his mask kisame knew that its Madara , since he was aware of his existence and it couldnt be any1 else with Sharingan since only Uchihas left are madara and sasuke.

Madara was the "Unkown Mizukage" Of Kirigakure....We dont know if he was the 1st or 2nd or 3rd ,but we know he was Mizukage...We also dont know why he quited from being Mizukage and stuff...Personally I Think That he was the 3rd Mizukage before te Jinchuriki Yagura(4th Mizukage) and THE Currrent one..
