Kotoamatsukami is not a "simple genjutsu". It is arguably the most powerful genjutsu. It's only counter is if you have a partner who knows about it. Otherwise, you're under the control of the caster and you wouldn't even know it.

Kaguya, being omniscient, may know about it and know that Shisui and Danzo want to use it on her in advance so could probably create Zetsu to break her out of it when she gets hit by it.

Kaguya, Madara and Sasuke have the Juubi Sharingan which would most likely allow them to resist KA. Naruto has 9 Bijuus in him which also is the reason perfect jins would be uneffected by it.

Rennigan has far stronger genjutsu than Shisui's eye's technique... so i don't think a genjutsu would work on a Rennigan user unless the opponent has a Rennigan as well.

Ao did not break it. He realised the 4th mizukage was being controlled with genjutsu. The genjutsu wasn't even cast on him. Besides the genjutsu he was talking about was obito's sharingan genjutsu not KA. Lol
