She builds up massive amounts of chakra in her fist and releases it all at once, pretty much explosions coming from her hands, obviously not the release but its still explosions.
Lee's primary lotus is nothing like Sakura's punch, lee is using speed to create powerful impact while Sakura is using her chakra in explosive waves. She most definitely knocks sand away.
