So how is a surgery with an intact spine more dangerous and improbable for success than literally having your body split into two pieces?

It was such a big deal on whether or not Lee would be able to live or stay a ninja. He even had to do physical therapy before being 100% again. Tsunade just shrugs it off like she has no fks to give and just goes back to the battlefield.

Not WTF at all. You should remember that when she was doing the intervention to Lee, she just went back to do her job as a medic (and also as a Hokage), because she spent a lot of years just gambling, meaning that the reason of the success rate of the operation was due her negligence and lack of practice.

If she is able to do these things now, well... it's obvious. She has been doing a good job as a kunoichi lately, she got the momentum of her abilities. And in the most extreme of cases, even she can get powerups, don't you think? Why would she get stuck in her improvements?

Can be used to stop bleeding lol ^ we use em all the time, only as a last resort though if the bleeding wont stop but thats the only logical thing I could see Kishi going with and yeah I figured some parts are done for but I'm sure she can bring some of them back.
tourniquets are used to stop the bleeding. either way, that tree obliterated everything that it smashed into. her organs are not intact from 10 feet separation of her top/lower half. just use reason man o_O

Its no plothole. Rock lee's broken bones and torn muscles had healed, but fragments of bone had lodged in his spinal column, and removing them would involve an extensive operation that only she was capable of, her cut was probably clean, otherwise she would not have been able to heal it. All she had to do was put herself back together
Was about to say the same. Also there is clearly a huge difference in what is considered dangerous to rock lees body and to tsunades.
