Jutsu sense and sharpness. His jutsu instincts and sharpness. Nowhere does it mention Kabuto being stronger than Tsunade, or even superior to her in medical ninjutsu for that matter.

First of all, we know that on both counts Tsunade is referring to Kabuto's "jutsu". Up until that point, the only ninjutsu Kabuto had shown Tsunade were his Chakura no Mesu (chakra scalpels). It follows that Tsunade would have been referring specifically to his use of chakra scalpels alone.
