I don't know how Japanese tradition goes but from what I've seen characters tend to take after their mother's surname, so if Sasuke does become a father won't he/she take after their mother's last name as well?

If you're talking abut Naruto its been explained that he got his moms last name from hiruzen to protect naruto from minatos enemies

This was only the case for Naruto because if they gave him Minato's surname then everyone would know he's the son of the 4th Hokage which would put him in danger. As the 4th Hokage had many enemies.

I sort of thought that (mother's name gets passed down), although it really isn't concrete enough imo. If only Hashirama's child was known/told and their last name. I doubt believe their were any other "mixed" families that I know of.

I don't know that the transmigrants have to be Uchiha and Senju (example, since the Senju are mostly extinct, an Uzumaki-Namikaze is the next incarnation) Uchiha were often the incarnation because they were naturally more powerful, also, we don't know if the incarnations were always Uchiha and Senju (Uchiha clan would take multiple generations to officially form even after Indra's death).

No. They don't take their mother's surnames. Naruto was given the surname 'Uzumaki' to protect him from people who would endanger him. Most likely the enemies of Minato. So no, Sasuke's children will be fine.
