Actually Sasuke went to Orochimaru to get lvl2 CM. Itachi obviously was going to give Sasuke his eyes, since, as you've pointed, he didn't crush them. Naruto had conquered Kyuubi first by powers and then with ideology, he was never given with it.

Kyuubi is not power itself, you have to conquer it in order to get it's power. So obviously Sasuke.

I don't know, but I think that all affinities combined = TBB. Rasengan was created based on Minato's observing TBB's, and he believed that to complete it some affinities must be added.
