Ok we see in this chapter, Madara unlocking his Mangekyo Sharingan. But nobody died. He lost nobody. But in fact, his best friend "Hashirama", died in his heart, because he's a senju. Thats why he unlocked it. So nobody has to die physicaly to unlock the MS. Emotional disturbance is enough.
