Hokage is the highest, but such a rank is meaningless. Itachi for example is above Kage level. Uchiha Madara is miles above Kage level, so it really comes down to specific skill rather then what rank you are. Remember Naruto is a Genin, yet he's above Kage level. Just saying xD
