I know that Kishi implies that the Rinnegan is an evolvement to the sharingan...but you cant make this assumption yet. Madara fought Hashirama so that he could get a sample of Hashirama, his goal was for the future...ie unlocking the Rinnegan. This may have been a factor in him unlocking...

He didn't lie to sasuke, it is lost in translation, it meant to say that he would be the 3rd person ALIVE who would have the Mangekyou sharingan(Madara, Itachi and Sasuke) kakashi didnt have his yet and Itachi didnt know he would unlock it.
