I made a thread earlier named "Zetsu's recordings was never meant for Tobi, they were for Madara Uchiha".
The Sasuke & Itachi fight was recorded and brought to Tobi which would mean Madara was alive at the time.
No other recordings have been stated after the battle of the Uchiha brothers. So it is now possible to presume Madara did infact die after that fight.
To keep his plan in motion he made Tobi the official leader. This is when Tobi changes from "good boy", errand boy to an certificated badass. (attitude & voice change.) He is now responsible to keep the Moon Eye plan in motion until Madara is revived by Nagato.
I find the upper part very likely.
This second part is just a thought which is not very likely and i don't really believe in it. But i'll share my thought.
Orochimaru might have had Madara's DNA all along without informing his subordinate Kabuto.
So why did he not summon him?
He tried. But failed due to the fact that Madara was still alive in his weak state. This is something not even Orochimaru could have foreseen.
It was believed to be the fourth hokage for years until it was finally confirmed to be him by Kabuto 300 chapters later. This is the only solid evidence mangavise that the third coffin was infact Minato, the fourth Hokage.
However Orochimaru is now back and is the only man that can disprove Kabuto.
Madara would be a better Edo tensei summon than Minato. So if he had access to him he obviously would have chosen him. Kabuto later steals every DNA sample that Orochimaru had including Madara's.
Manga
In the manga the coffin fails to summon altogether. It simply did not work.
Anime
In the anime the coffin comes up but then fails. The coffin also says "fourth" which is weird since it was revealed that it was the fourth 300 chapters later. (not canon)
I find the lower part more farfetched and less likely.
